Protective Life Foundation
Criteria for Submitted Proposals

(Important note:  Please review the Protective Life Foundation's Contribution Guidelines prior to preparing a proposal.)

Please provide the following information when submitting a request to the Protective Life Foundation. Failure to include all relevant material may result in the exclusion of the request from the Foundation's review cycle. IMPORTANT: The organization submitting a grant to the Protective Life Foundation must be a 501(c)(3) organization as designated by the Internal Revenue Service.

I.     Cover Letter (Make sure to include an e-mail address.)
  Please include a cover letter (maximum of 4 pages) that includes the following information:
A.  Name, address, phone, fax, e-mail and contact person of the organization seeking the grant.
B.  Mission/brief overview of the organization.
C.  Intended use of the request.
D.  Project description and evaluation.
E.  Who is served?  How many are reached each year?
F.  Specific amount of request - one year or multi-year?
G.  Accountability of program - how is the "success" measured?
H.  Uniqueness of the organization - what makes the organization different from any other like entity?  Are there any other local or regional organizations that do similar work?
II. Financials (attachments in addition to the cover letter)
A.  Income and expense statement from most recent fiscal year.
B.  Project budget, including a line item income and expense budget.
C.  Does the organization have any reserves, endowment funds or any other sources of revenue (city, state, federal, United Way, etc.) other than private corporate and individual donors?  If so, please explain.
D.  List of other corporations/foundations being solicited and the status of each request.
E.  Other supporters and amount of gift(s).
III. General Organization (attachments in addition to the cover letter)
A.  Executive Director's experience/brief bio.
B.  Number of full-time & part-time staff members.
C.  List of Board of Directors (Do they contribute to the organization?).
D.  Copy of tax-exempt letter from the IRS.

Depending on the nature and amount of the request, some proposals will be reviewed monthly, and others will be considered on a quarterly basis.  The proposals will be reviewed by the Foundation's Executive Committee, and all grant recipients must be charitable organizations as designated by the IRS under Section 501(c)(3) of the Internal Revenue Code.

Quarterly Review Cycle:  March, June, September and December
