    MONY or MLOA Life Insurance and Annuity Claims

    To report a death on a MONY or MLOA Life Insurance policy or Annuity contract, call our Claims Service Center at 1-800-659-1058. The Claims Service Center is open Monday through Friday 8 a.m. to 5 p.m. EST.

    The Insurance Claims Process

    Protective Life believes that caring for beneficiaries is at the heart of our purpose as a company. That’s why you’ll find making a life insurance claim with us is as easy as following these four steps:

    Step 1: Notification

    First, let us know of the loss of your loved one by downloading, completing and mailing the claim packet or calling us at 1-800-424-1592.

    Step 2: Filing a Claim

    The life insurance claim packet will ask for basic information regarding your claim. You can obtain this packet by mail when you notify us of the passing of your loved one by phone, or you can download a claim packet on this website. Once you have the packet, please complete all information and make sure to include a certified death certificate.

    Step 3: Receiving Your Funds

    Once your life insurance claim is approved, your benefit payment will be sent out within one business day.

    In the claim packet, you may be given a choice of how you want to receive your benefit payment. If your benefit amount is $10,000 or higher you may be eligible to receive an Immediate Benefit Account. The Immediate Benefit Account offers the convenience of an interest-bearing draft account – similar to a checking account – for your funds instead of a one-time, lump-sum check. You can receive a personal checkbook and write checks to third parties, including individuals, stores, creditors, banks, brokerage firms and other financial institutions. You can also write a check to withdraw your entire balance and close your account at any time.

    Step 4: Financial Counseling

    After you receive your benefit payment, we recommend contacting your financial planner or Protective to discuss options regarding how to manage your proceeds.
